ADMISSION CRITERIA

INTAKE SERVICES

The intake services are closely coordinated with the Native Community Worker who will be required to participate in treatment preparation, support and aftercare programming. Telephone referrals are not accepted; however we will assist those interested in obtaining the written materials required for referral.

Due to the length of the healing program, the intake process should begin as early as possible. The referral will be assessed according to our admission criteria. Most children and youth will be seen within four weeks of referral confirmation for an interview to determine how their needs can best be met by our centre.

Intake Procedures:

Complete the following forms:

  • Pre-assessment Questionnaire
  • Medical Assessment
  • Referral Agent Agreement
  • Liability Waiver
  • Parent Agreement for Treatment
  • Consent for Medical Treatment
  • Consent for Disclosure of Information
  • Consent for Alternative Education

Confirmation of space will only be made upon receipt of all referral forms.

If the referral forms are not received or alternative arrangements have been made under special circumstances for medical assessments one week prior to admission, we reserve the right to cancel the clients space and fill it with someone from the waiting list.

If the client changes his / her mind about treatment, please telephone Nimkee NupiGawagan Healing Centre as soon as possible.

If a client is referred within one week prior to admission day, confirmation of acceptance will be made by phone or fax.

Clients that have submitted referral forms are not necessarily guaranteed acceptance, therefore parents and referral workers should not assume automatic acceptance. Confirmation of acceptance into the program will be made by telephone and by letter from Nimkee NupiGawagan Healing Centre.

All children / youth must be escorted to and from the Program by an recognized Child Care representative (NNADAP worker, Child and Family Services, or equivalent agency), with the parents or having authorizations from a parent / guardian.

Referrals will only be considered when:

  • All referral forms have to be completed
  • All questions are answered on the forms
  • The forms are appropriately signed by the parent / guardian and referral worker
  • Do not hold up the referral forms while waiting for the medical assessment to be compiled. Send the referral forms as soon as possible and note that the medical assessment will be sent later.
